You Do not have to fill out all the medical forms Amazon asks for to request an accommodation.

Amazon has been requesting too much medical information. Their forms are confusing and are intrusive.


Amazon wants you to have your doctor fill in Amazon medical forms, but you don't have to have your doctor fill in Amazon forms. Your doctor can simply write a letter (and It can be short) and you can file an accommodation request with that letter.


We suspect that the forms Amazon asks you to give to your doctor to fill out are looking for information to help Amazon in denying your request.


It's better and several Amazonian's have simply submitted a doctor note regarding your disability and accommodation needs.

For example multiple individuals with disabilities simply had their doctor write a 2 sentence letter for remote accommodation.

Amazon plays a very insidious game where the information they request appears as part of some standard compliance, but it's not. There is only so much information they can require of you to make a decision.


They do not need entire pages or forms of medical documentation. The ADA allows for you to simply provide a doctor note stating your accommodation needs.


In other words, never use Amazon medical forms when submitting a accommodation request.


What you can do when you get asked to provide too much medical documentation is:
Send an email with something similar to this to:

"I'm concerned that the form Amazon asked me to fill out when requesting an accommodation request access to my personal medical information beyond what's required by the ADA to assess my accommodation needs. I intend to provide a doctor note regarding my accommodation needs and limitations and believe this is sufficient under the ADA. If there are any key questions that are required by law on the Amazon accommodation forms, please send them to me via email here and as long as they are required by law I will provide them with my doctor. Please confirm receipt of this email and my explanation, and confirm that what I stated is acceptable."
